Transgender Athletes in Women’s Sports: The Search for a Fair and Workable Policy





Few issues in sports produce more heat—or less productive conversation—than the participation of transgender women in female competition.

That’s partly because the debate touches several sensitive questions at once. What makes competition fair? When does a physical advantage become too significant? How should sports balance gender identity with sex-linked athletic differences? And when an event involves tackling, striking, or collision, how much uncertainty about safety is acceptable?

The language matters, too. Transgender women are people whose gender identity is female and whose sex was recorded as male at birth. Describing the issue only as “biological males competing against women” highlights concern about sex-linked performance differences, but it can also dismiss the identity and dignity of transgender athletes.

A serious discussion has to hold two ideas at once: transgender athletes deserve respect, and female athletes deserve rules that preserve meaningful competition.

The central question isn’t whether transgender people should be allowed to play sports. It’s whether—and under what conditions—transgender women who experienced male puberty should compete in female categories, particularly in elite, strength-dependent, endurance, combat, and collision sports.

Why Women’s Categories Exist

Sports accept all kinds of natural advantages. Elite athletes differ in height, limb length, aerobic capacity, reaction time, muscle composition, and access to coaching. Nobody proposes separate basketball leagues for tall and short players.

Sex is treated differently because the performance gap that emerges after male puberty is large, consistent, and relevant across many events.

A 2021 review by Emma Hilton and Tommy Lundberg reported that performance differences between males and females become pronounced during puberty. Depending on the sport and the measure being studied, the gap can range from roughly 10 to 50 percent. It is especially large in activities that depend on muscle mass, explosive strength, and upper-body power (Sports Medicine).

These averages don’t mean every male athlete will beat every female athlete. A trained woman can easily outperform an untrained man. But sports policy can’t be built around isolated comparisons. It has to account for population-level differences and what happens at the highest levels of competition.

There, even a tiny advantage matters. Fractions of a second can separate a medalist from someone who doesn’t qualify. An advantage doesn’t have to guarantee victory to change who reaches a final, earns a scholarship, breaks a record, or makes a national team.

Female categories exist because an unrestricted category based only on performance would leave relatively few women represented at the top of many sports. The harder question is whether hormone treatment changes the effects of male puberty enough to make competition fair.

What Hormone Treatment Changes—and What It Doesn’t

Gender-affirming hormone therapy causes real physiological changes. Suppressing testosterone can reduce hemoglobin, lean body mass, muscle area, and strength. The size and timing of those changes vary, and different traits respond at different rates.

Hilton and Lundberg’s review found that lean body mass, muscle area, and strength generally declined by about five percent after 12 months of testosterone suppression. The authors argued that this reduction may leave a meaningful share of the muscular advantage gained during male puberty intact (Sports Medicine).

A broader 2023 review by Nokoff and colleagues stressed that athletic performance depends on many interacting factors: hemoglobin, muscle mass, strength, body size, biomechanics, training, and the specific demands of each sport. The researchers called for more long-term studies involving trained transgender athletes rather than relying mostly on sedentary participants or indirect measures (Exercise and Sport Sciences Reviews).

That distinction matters. Lowering testosterone doesn’t reverse every effect of puberty. Adult height, skeletal dimensions, limb length, joint geometry, and some muscle-structure features may remain. At the same time, reductions in testosterone and hemoglobin can hurt performance, especially in endurance events.

So two common claims miss the mark. It isn’t accurate to say hormone treatment changes nothing. It also isn’t accurate to assume that reaching a testosterone threshold removes every relevant advantage.

The research faces a practical obstacle: relatively few transgender women compete at an elite level. That makes large, sport-specific studies difficult. Many studies measure health or fitness indicators rather than competition results, and some involve participants who aren’t trained athletes. Findings from those studies can’t automatically be applied to Olympic events, professional leagues, or contact sports.

Fairness Depends on the Sport

“Athletic advantage” is often discussed as though every sport rewards the same body and abilities. It doesn’t.

Basketball rewards height and reach. Weightlifting depends heavily on strength and explosive power. Distance running places greater emphasis on aerobic capacity and running economy. Shooting and archery rely more on precision, stability, and concentration.

Hamilton and colleagues argued that transgender women may be integrated more easily into elite archery and shooting because the traits behind large sex differences in strength- and power-based sports may matter far less in precision events (European Journal of Sport Science).

That makes a universal rule hard to defend. A policy designed for boxing may make no sense in shooting. A standard appropriate for the Olympics may be needlessly strict for a local recreational league.

Each governing body should ask a few basic questions. Which sex-linked traits predict success in this sport? How much do those traits change during hormone treatment? Do any remaining differences matter at this level of competition? And do they affect only results, or could they also put opponents at greater risk?

Safety Changes the Calculation

Competitive fairness and physical safety aren’t the same issue.

In running or swimming, a retained advantage may affect placement or qualification. In boxing, rugby, martial arts, and other collision sports, differences in mass, strength, reach, speed, and force may also affect the likelihood or severity of injury.

This doesn’t mean every transgender woman is dangerous to female opponents. Nor does it mean an injury is inevitable. The concern is simpler: policy mistakes can carry more serious consequences when athletes deliberately strike, tackle, or collide with one another.

Direct research on injury rates involving transgender athletes remains limited. Jon Pike’s analysis of rugby argued that the safety question requires evidence showing whether transgender participation raises injury risk and whether any increase is large enough to justify exclusion (Journal of the Philosophy of Sport). Joanna Harper’s examination of international policy similarly noted that some rugby injury calculations relied heavily on assumptions rather than extensive observed data involving transgender players (Law and Contemporary Problems).

The lack of evidence cuts both ways. Presenting a precise injury risk as settled fact would be misleading. It would also be reckless to assume there is no additional risk simply because large datasets don’t yet exist.

Combat sports already use weight classes because physical differences affect fairness and safety. Yet equal weight doesn’t mean equal physical capacity. Two athletes in the same weight class can differ in reach, skeletal structure, muscle distribution, speed, and striking force. That’s one reason combat and collision sports may need stricter eligibility standards than noncontact events.

Inclusion Has Consequences Too

A discussion focused entirely on physiology misses the human cost.

Transgender people often encounter hostility, bullying, and exclusion in sports. A 2024 systematic review and meta-analysis found that discrimination in athletic settings was associated with mental-health burdens among transgender athletes and identified recurring barriers to participation (BMC Psychology).

Research involving transgender girls has also shown how poor treatment at school and in sports can drive them away from athletics. Some participants said they avoided teams because joining could expose them to further rejection—even though sports can offer friendship, confidence, exercise, and better mental health (Transgender Health).

None of this settles eligibility for elite female competition. It does show that exclusion isn’t cost-free. Rules can protect a competitive category without humiliating athletes, exposing private medical information, or portraying transgender people as threats.

Age and competitive level matter, too. Elementary-school recreation isn’t the same as a professional title fight. Youth sports often emphasize exercise, belonging, and skill development. Elite sports distribute medals, records, scholarships, contracts, and national-team positions.

A rule applied identically to both settings will almost certainly be too permissive in some cases and unnecessarily harsh in others.

What’s at Stake for Women’s Sports

If governing bodies allow athletes who experienced male puberty to enter female categories without meaningful restrictions, female athletes may face stronger competition for roster spots, records, scholarships, prize money, and titles in certain sports.

The number of transgender competitors doesn’t have to be large for the effect to matter. Elite opportunities are scarce. One athlete can influence who reaches a podium, makes a team, earns funding, or holds a record.

Trust matters, too. Athletes and spectators need to believe that the rules create a credible contest. If female athletes conclude that their category no longer accounts for the performance effects of male puberty, participation and public support could decline. That concern shouldn’t be dismissed simply because transgender participation remains rare.

Blanket exclusion creates its own problems. It may push transgender people out of organized exercise, encourage invasive sex verification, and produce rules that also affect athletes with differences in sex development. Poorly designed policies can invite suspicion and harassment toward any woman whose body or appearance doesn’t meet conventional expectations.

The answer isn’t to pretend sex has no relevance in sports. Nor is it to treat every transgender athlete, sport, and competitive setting as identical.

A workable policy has to account for the sport, age group, competitive level, puberty history, and degree of physical contact.

A More Defensible Approach

At the elite level, governing bodies should begin with the reason the female category exists: to preserve fair opportunities for athletes whose performance hasn’t been shaped by male puberty.

Where credible evidence indicates that the effects of male puberty remain competitively significant, eligibility restrictions may be justified. The case is strongest in sports that depend heavily on strength, power, speed, or physical contact.

That doesn’t have to mean excluding transgender athletes from sports altogether. Depending on participation levels and practical constraints, organizations could test open categories, mixed events, expanded recreational divisions, or other competition formats.

An open category isn’t a magic fix. It may be difficult to sustain if too few athletes enter, and smaller organizations may lack the resources to create additional divisions. Still, it’s worth exploring rather than assuming every athlete must fit within the same two-category structure.

Precision-based and low-contact sports may permit broader inclusion when the retained effects of male puberty have little relevance to performance or safety. Youth and recreational leagues may reasonably weigh participation and belonging more heavily, provided they address serious safety concerns. Elite championships can impose a stricter standard because their purpose and stakes are different.

Whatever rules organizations adopt, they should be transparent and open to revision. Governing bodies should publish the evidence behind their policies, acknowledge uncertainty, collect performance and injury data, and update eligibility standards when better research becomes available.

Privacy and respect shouldn’t be optional. Athletes must not be publicly humiliated, subjected to unnecessary medical scrutiny, or treated as political symbols.

The Honest Answer

The science gives legitimate reasons to worry about fairness in at least some female sports. Male puberty produces average advantages in size, strength, power, and other performance-related traits. Testosterone suppression can reduce some of those advantages, but it doesn’t necessarily erase all of them.

How much that matters depends on the event.

Safety calls for added caution in combat and collision sports, though claims about exact injury risks shouldn’t outrun the available evidence. Inclusion matters too. Transgender athletes can suffer real harm when policies are demeaning, indiscriminate, or designed with no regard for participation and mental health.

The debate is often framed as a choice between protecting women and respecting transgender people. That framing is too crude. Sports organizations can recognize transgender people’s dignity while maintaining sex-based categories where those categories are necessary for fair and safe competition.

Good policy won’t come from slogans or a single rule forced onto every sport. It will come from asking a narrower, more useful question:

What rules make sense for this sport, at this level, for these athletes?

That answer may not satisfy people demanding a universal policy. It has a better chance of preserving both meaningful competition and humane treatment.
